HOW TO GET CLEARED TO VOLUNTEER AT SEASIDE

If you are new to Seaside, we need

  1. The attached 4 page volunteer form filled out and signed
  2. A copy of your Driver’s License (to process your Megan’s Law clearance)
  3. A copy of your cleared TB test result
    Or New this Year, a Certificate of Completion of the TB Risk Assessment Questionnaire signed by your doctor (available online at http://www.cdph.ca.gov/programs/tb/Documents/TBCB-Adult-TB-Risk-Assessment-for-schools.pdf)
  4. To take your photo to make your badge

If you are continuing at Seaside, to make the process go smoother and quicker we need the following:

  1. The attached 4 page volunteer form filled out and signed
  2. A copy of your Driver’s License (to process your Megan’s Law clearance)
  3. TB Tests are good for 4 years, please attach a copy of your current valid TB Test. If it is expired, please submit a new TB Test result. Or New this Year, a Certificate of Completion of the TB Risk Assessment Questionnaire signed by your doctor (available online at http://www.cdph.ca.gov/programs/tb/Documents/TBCB-Adult-TB-Risk-Assessment-for-schools.pdf)
  4. You can still use your badge from this year, you will need a 2015-2016 sticker

Please submit all paperwork together (volunteer form, TB test, and copy of driver’s license). An envelope will be in the office to collect the forms. If you do not attach all the requested items, you will be asked to provide the additional missing items. Your paperwork will be submitted for clearance once you have provided everything required.

As applications are cleared, a list will be posted in the office and on the Seaside website. You will not receive your badge until you receive your clearance.


Please DO NOT bring your electronic recycling to Seaside

We are no longer accepting recyclable items (batteries, cell phones and ink cartridges) for drop-off in the computer room. Staples office center accepts rechargeable batteries, cell phones, small electronics and ink cartridges, please see their website for details. Alkaline (non-rechargeable batteries) and many other items can be recycled at the Gaffey Street S.A.F.E. Collection Center at 1400 N.
